Our Criteria
In order to qualify for our Housing Stabilization Services, individuals need to fulfill the following eligibility requirements:

Must be enrolled in Medical Assistance (MA)

Must be at least 18 years old

Have a disability or disabling condition

Must require assistance in at least one of the following areas due to a disability: Communication, Mobility, Decision making, Managing challenging behaviors

Must have experienced housing instability, which includes homelessness, transitioning from an institution, or being at risk homelessness or institutionalization.
